Lakeland Regional Health is searching for an experienced, full-time, Director of Case Management. This role is responsible for developing and implementing processes to ensure that patients move through the continuum of care efficiently and effectively. This position includes oversight of the social work/case management department, integrated care team, utilization management, and contracts with post-acute care providers and transportation services. The leader will assess and implement process improvements to optimize resource in patient flow, discharge planning, post-acute patient placement, length of stay management, readmission reductions, and CMS bundle payment programs.

What You Will Need:
Qualified candidates will have a Master’s Degree from an accredited school in Social Work, Nursing, Healthcare Administration, or Business Administration. A minimum of 5 years of experience within healthcare delivery systems in case management, social work, nursing, or care continuum are required. Other essential skills include demonstrated analytical and diagnostic skills in optimizing patient flow and care continuum, broad knowledge of community resources, and a strong understanding of payor plans and reimbursements. We are looking for a solid decision maker, charismatic communicator, and great relationship manager.
