The Office of Care Management at Dartmouth-Hitchcock (D-H) has career opportunities for Case Managers. Qualified candidates will possess both recent acute hospital care experience and Case Management experience. D-H Case Managers manage complex discharge plans, while collaborating with a team of nurses, social workers and providers on the patient’s behalf. These positions require excellent communication skills with the ability to educate family members and patients on their discharge plans, engage in difficult conversations around patient care and have the network of support services secured prior to discharge. Qualified candidates will have well-developed critical thinking skills and be able to develop parallel discharge plans to be prepared for the unexpected.
A Bachelor’s degree with 3 years of clinical experience required.
Recent acute care hospital experience preferred.
1 year Case Management experience.
Strong communication and computer skills desired.
Required Licensure/Certification Skills:
A Bachelor’s degree
Licensed Registered Nurse in New Hampshire required or eligibility for NH license.
Eligibility Criteria for Sign-On bonus:
Must possess valid unrestricted Registered Nurse licensure to practice nursing in New Hampshire (or be eligible). Must have one year of Case Management experience in an inpatient setting. Must not have been employed by D-H or D-H member within a recent six-month period. Hire effective date on or after October 1, 2018. Program may be modified or terminated at the discretion of D-H.
Participant must accept a Full-Time position – Minimum 30 hours per week/60 hour bi-weekly. Participant is required to authorize an obligatory promissory note to complete work agreement. Repayment required upon default. Full bonus payment provided upon first employee paycheck. A 2-year work commitment is required.
