The Case Manager is responsible to the Director of Case Management for the coordination of care of a selected group of patients to achieve optimal outcomes with effective resource utilization from admission through discharge. The Case Manager exhibits a leadership role of influence, collaborates with interdisciplinary team members both internally and externally, and ensures continuous, efficient, quality care.
