Nurse Case Manager

Our growing Hospice team has an immediate opening for a full-time Registered Nurse (RN) Primary Care to provide direct nursing care to patients and support to their family members.  JSSA Hospice is not only a supportive team for all of our families, but we are supportive of our team members.  Our professionals, experts in current hospice practice, include medical directors/physicians, highly skilled registered nurses, experienced clinical social workers, certified nursing assistants skilled in end-of-life care, chaplains and extensively trained volunteers.  For over 30 years, our hospice has offered a wide range of quality medical and support services.  We are the hospice in the community to turn to for exceptional end-of-life care that supports dignity and comfort for individuals, provides peace of mind to families, and honors cultural traditions and customs. 

General Overview:

• Full- time Primary Care RNs are responsible for carrying a caseload of approximately 12 patients.
• Identify and evaluate palliative needs of patients, utilizing the nursing process; with particular attention to pain assessment and response to treatment.
• Render age appropriate skilled nursing care in accordance with the plan of care using current and accepted nursing knowledge, methods and techniques.
• Instruct and counsel family members and other caregivers in hospice care (including interpreting the plan of care).
• Have sufficient knowledge of third party reimbursement, especially Medicare/Medicaid, to interpret to the patient and family the availability and/or limitations of these programs.
