Summary

Coordinates the care needs of assigned patients and develops comprehensive plans to manage care delivery across the patient care continuum.
Partners with patients and their primary physicians to develop customized care plans based on their individual needs and preferences.

Responsibilities

  • Collaborates with physicians, nurses, allied health professionals, social work, and others to ensure appropriate tests and treatments are delivered in a timely fashion.
  • Advocates for the patient.
  • Balances care needs and financial considerations to ensure efficient and effective treatments are achieved.


Qualifications:

Education

Required: Academic degree in nursing
Preferred: Bachelors or Masters degree

Experience

Required: 2 years nursing experience.

Certifications

Required: Must be currently licensed, certified or registered to practice profession as required by law, regulation in state of practice or policy. BLS
