Serves in an expanded role to ensure delivery of comprehensive, high quality and cost-effective care with appropriate allocation of medical resources to Geisinger at Home.
Assists the health care team (providers, clinic nurses, and members) with access to community resources and other care coordination activities including protocol-driven disease management education for Geisinger at Home patients. Collaborates with the patients, Geisinger at Home Team, and primary care providers in multiple practice sites to facilitate clinical guideline adherence and coordinate services specifically related to the members health care needs. Follows plan of care for assessed member needs, education pathways, clinical guidelines and diagnostic tests to assist the member and primary care team with individualized plans of care. Assesses, plans, implements, monitors and evaluates options and services for members admitted to acute and subacute facilities, receiving home health services or those independent at home.
Reports through the Geisinger at Home Operations Manager.

Demonstrates general computer knowledge, with proven keyboarding ability required.
EDUCATION AND/OR EXPERIENCE:
Current licensure as a Licensed Practical Nurse in the state of Pennsylvania is required.
Minimum of five years acute care nursing experience, with no less than two years in an acute care hospital setting, which preferably includes intensive care, critical care or Emergency Department experience. Case management or Utilization/Medical Management experience preferred, as is knowledge of basic concepts and principles of managed care.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Work is typically performed in an office or clinical environment. Valid Pennsylvania driver's license and a willingness to travel required, using company or personal vehicle required.
