Advanced Practice Registered Nurses / Physician Assistants

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Provide patient care with a focus on sickle cell and intellectual disability
Collaborate and coordinate patient infusion needs with the CCHS Infusion center at Wilmington Hospital and Christiana Hospital
Provide infusion center and urgent visits
Collaborate and coordinate annual wellness visits and post hospital discharge follow-up visits
Assess and document patient and family response to intervention
Demonstrate ability to carry out critical skill as outlined in procedures and specific standards
Prescribe medications in a safe and efficient manner
Establish priorities based on patient needs and emergent care
Independently review patient problems including, but not limited to medication refills, pain management, determining the need of follow up care and referring appropriately to the physician according to policy
Perform all prescribed laboratory test, send to appropriate laboratory, receive and interpret laboratory values, and notify physician of results
Provide patient education and counseling to patients and their families
Manage patient's chronic disease inpatient with collaboration of hematologist
EDUCATION AND EXPERIENCE REQUIREMENTS:
Master's degree in Nursing with Family Nurse Practitioner Board Certification or graduate of an Accredited Physician Assistant program.
1 year of experience as Nurse Practitioner or a Physician Assistant preferred
Licensed or eligible for license in the State of Delaware
