Advanced Practice Registered Nurses / Physician Assistants
The Nurse Practitioner (NP-S) practices as a memberof the interdisciplinary healthcare team using standardized procedures toindependently examine, diagnose and treat patients in collaboration withsupervising physician(s).
Expert ClinicalPractice and Collaboration
A.Independentlyexamines, diagnoses, orders diagnostic studies and treatment for patients usingapproved standardized procedures. Evaluatesthe effectiveness of interventions and changes treatment as indicated.
B.Provides direct patient/indirect care.
Coordinates,prioritizes and organizes individual patient care in collaboration withsupervising physician(s
Collaborates withmembers of the healthcare team to create a patient plan of care.
Providesindividualized patient and family education and documents in the patientteaching plan.
Communicates withsupervising physician and physician consultants treating the patient.
Anticipatesdischarge on admission and plans accordingly.
Ensures accuratepatient problem list and medication reconciliation on admission and discharge.
Utilizes agespecific competencies for culturally diverse patients.
Ensure accuracy ofacuity with appropriate diagnostic documentation.
Ensures a seamlesstransition through the hospital continuum of care. May follow in a outpatientclinic or physician's office.
May practice in a clinic, inpatient setting and/or outpatient setting.
