Care Navigator

• Create connections between vulnerable populations and health care systems.
• Manage care and care transitions for vulnerable populations.
• Ensure cultural competence among health care professionals serving vulnerable populations.
• Provide culturally appropriate health education on topics related to chronic disease prevention, physical activity, optimal functioning and management of activities of daily living and nutrition.
• Advocate for assigned underserved beneficiaries to receive appropriate health care and community services.
• Build capacity for each beneficiary to maximize their health, behavioral health and every day functional capacity.
• Outreaches (telephonically and in person through potential home visits as needed to the Special Needs population) to each of their assigned lower risk beneficiaries.
• Provide and coordinate services such as a Health Risk Assessment.
• Member education, advocacy and coordination of beneficiaries care are essential services the member will receive from their navigator.
• Responsible for a panel of approximately 200 beneficiaries to arrange and coordinate services for and ascertain that each and every assigned beneficiary has had a HRA completed within 90 days of enrollment.
• Assess and coordinate with other team members that the beneficiary is making progress towards the goals as stated in the individual care plan.

QUALIFICATIONS

- EDUCATION - H.S. Diploma/GED Equivalent

- EXPERIENCE - A minimum of 1 year experience required
