Patient Navigator RN, Full time Day, Murmur Clinic, Abrazo Arizona Heart Hospital

The RN Navigator enhances the patient experience by interfacing with various patient care teams and assists to provide comprehensive care to patients who are in need of diagnostic and therapeutic procedures, and those newly diagnosed with medical conditions requiring treatment/follow-up. The RN Navigator works collaboratively with physician specialists (both interventional and diagnostic), genetic counselors, referring physicians, other treating physicians, surgeons and other treatment navigators to ensure that the patient gets seamless care for optimum outcomes. This position provides clinical functions as required in support of services and participates in maintaining a safe, cost-effective patient care environment. The RN Navigator also serves as a community liaison for education and awareness.

May require the knowledge and skills necessary to provide various services, depending on the work setting. Able to navigate patients through various multidisciplinary programs. Excellent problem solving skills. Strong written/verbal communication and interpersonal skills to effectively interact with co-workers, referring physician groups, and radiologists
