Description Job Summary: As a Registered Nurse Case Manager in the Transition Management Office you will be expected to use critical thinking skills, based on your nursing assessment, while working with the clinical care team, to ensure progression of the patient's plan of care through transition. This role has accountability for a patient's reasonable, and appropriate transition through the continuum of care, assisting patients and families in establishing patient specific long-term and short-term goals, removing barriers and appropriately manage efficient and effective use of resources. The RN Case Manager provides leadership to the health care team to coordinate patient and family interventions and is the catalyst that identifies indicators for post-acute care utilization, referring patients (within the designated patient population) to the appropriate next level or site of care. The RN Case Manager serves in an advocacy role on behalf of patients, families, and caregivers to ensure equitable healthcare provision and assists in developing and meeting established outcomes for patients, as well as institutional goals, which among others will include customer satisfaction, and institutional financial objectives. $10k sign-on bonus. Vanderbilt University Medical Center (VUMC) is accepting applications for a RN Case Manager role within our Transition Management Office (TMO). The TMO administrative office is located in Medical Center North, on campus serving VUMC. Transition management or commonly referred to as care coordination is a thriving department comprised of nurses, social workers and support personnel. Currently there are 150 staff however the department has increased their staff needs to enhance the patient experience at Vanderbilt. The Vice President of TMO is Marcia Colone, Ph.D. who has over 25 years of experience in care management.
