VA Northern Indiana Healthcare System is accepting applications for an Advance Practice Registered Nurse (APRN). Selectees will work independently during clinic hours in the Primary Care (PACT) unit. THIS IS AN OPEN CONTINUOUS ANNOUNCEMENT. Applications will be accepted until the positions are filled and/or the closing date, whichever event occurs first; consequently, the position may be filled before the posted closing date. The Primary Care NP assists with the total care of the patient's medical needs by performing history and physical exams, ordering appropriate lab tests, and evaluating those tests, to ensure quality care of the patient. This will also include providing preventive medicine services, managing chronic disease problems and providing outpatient follow-up as needed. Duties include, but are not limited to: Performing initial medical histories and physical examinations. Providing and coordinating comprehensive care for assigned patients. Managing medical problems to include writing orders for medications for stable medical conditions and treating minor illness; ordering laboratory tests; ordering imaging tests; adhering diets and over-the-counter medications and supplies; reviewing diagnostic tests; reports and consults. Observing and recording patient's medical progress, updating and summarizing medical records, and changing orders when appropriate. Notifying the responsible Physician of significant changes in the patient's condition, in such cases there must be documentation of consultation with the supervising Physician. 'Notes and Orders' do not routinely have to be co-signed. The Nurse Practitioner may be delegated to document the discharge summary, but the supervising Physician must write the discharge note or co-sign the discharge summary. Write prescriptions in accordance with Federal and State law and as allowed by the Scope of Practice. Acting of clinical reminders. Performing medication reconciliation. Carrying out health promotion and disease prevention activities, including education or patient, family, and staff. Work Schedule: Mon - Fri 8:00 - 4:30P Financial Disclosure Report: Not required This position is eligible for the Education Debt Reduction Program (EDRP), a student loan reimbursement program. If selected for this position, you must meet specific individual eligibility requirements and submit your application within four months of appointment. The final award amount and eligibility period (one to five years) is determined by the VHA Education Loan Repayment Services office.
