The Iowa City VA Health Care System (ICVAHCS) is currently recruiting for one (1) Full-Time Registered Nurse- Rehabilitation & Extended Care Float Case Manager. ICVAHCS is in Veterans Integrated Service Network (VISN) 23. The Rehabilitation & Extended Care Case Manager Registered Nurse falls under the clinical pathway, as a direct care nurse, and reports to the Clinical Manager, Rehabilitation & Extended Care ICC. The Registered Nurse- Rehabilitation & Extended Care Float Case Manager functions as a case manager, with advanced clinical knowledge in the care facilitation of the Rehabilitation & Extended Care specialty Veteran. Has competence to function independently with little to no supervisor. Duties include but are not limited to: Identify patient care needs consistent with nursing practice. Receives and processes referrals for the specialty care management, including screening for referrals to the correct specialty service as needed and discussing with the specialty providers for acuity. Assesses patient for actual or potential nursing issues and complications. Works with the patient and family/caregiver to ensure understanding of the specialty care plan. Coordinates nursing care across the continuum, ensuring seamless transitions in care from outpatient through post-surgical or medical discharge. Follows up on referrals, labs and other tests to ensure continuity of care. Ability to cross-cover all the necessary clinic areas (such as Pain Clinic, Hospice/Palliative Care, Whole Health, Geriatrics, Spinal Cord, ALS, Polytrauma). Works cooperatively as a member of a team in all assignments. Work Schedule: Monday- Friday, 7:30 a.m. to 4:00 p.m. or 8:00 a.m. - 4:30 p.m. Financial Disclosure Report: Not required
