This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The Geriatrics and Extended Care (GEC) Nurse Practitioner Manager will be primarily assigned to the Hospital In Home (HIH) program, but may provide coverage to other GEC programs as needed. The Nurse Practitioner HIH Program Manager is the clinical and program manager for all aspects of the HIH. Duties: Serves as program manager for all aspects of the HIH program to include: policies and practice, budgetary and administrative oversight of compliance with Veterans Health Administration directives. Provides direct supervision of RN HIH Staff (currently 4) including performance appraisals, time and leave, schedules and any Human Resources actions including hiring, grievances and/or disciplinary actions. Provides program oversight including determination of program eligibility, reviewing HIH primary referrals, consult management with an emphasis on decreasing repeat hospital admissions. Utilizes critical judgment in the performance of comprehensive health assessments, different diagnosis, and the prescribing of pharmacological and non-pharmacological treatments in the direct management of acute and chronic illness and diseases. Manages and treats patients in their home setting until stable. Monitors program operations and metrics for regarding patient readmission. Meets weekly with HIH Staff to discuss treatment plans and execution of comprehensive education of disease processes in order to maintain patient goal of living independently. Serves as a resource to other members of the health care team, providing consultation and education. Demonstrates leadership through development and coordination of program to improve access to care, quality of care and organizational processes. Participates in interdisciplinary team meetings and performance improvement activities as determined by the GEC Chief, Chief of Staff or designee. Work Schedule: The work schedule will be determined based on the needs of the Medical Center. The Providence VA Medical Center is an acute care facility operating 24 hours a day 7 days a week. While most tours of duty are scheduled during the day, the possibility of working evenings, nights and weekends does exist. Shift changes will be considered based on patient and facility needs.
