This position is open only to employees of Tennessee Valley Healthcare System Telework available after 1 year of successful performance, if desired. Position is located at Liberty Square, located at 220 Glenis Drive, Murfreesboro. The RN in Remote Patient Monitoring-Home Telehealth (RPM-HT) will be responsible for the direct care of a panel of patients throughout the continuum of care to assure that care is timely, appropriate, of high quality and cost effective. The RN is primarily and accountable for assessing, planning, implementing, and evaluating direct and indirect nursing care through telehealth technology and identifying outcomes for patients in the remote patient monitoring-home telehealth program. In addition, the care coordinator provides ongoing evaluation of care management services. The RN is responsible for coordinating patient care delivery to ensure quality care according to established standards of care/practice, policy and procedure. The specific patient population to whom direct care is provided includes adults with different conditions. The nurse must have computer skills and telephone etiquette and a sound medical/mental health background to assess patient's data and make sound clinical judgments based on the data and speaking to the patients on the telephone. The nurse needs to become familiar with the equipment and how it works in order to assist patients in troubleshooting over the telephone when there is equipment failure. The practice of the nurse is characterized by leadership in improving patient care. The RN communicates regularly with nurse managers, physicians, nursing supervisors, charge nurses, transfer coordinators, allied healthcare staff, healthcare technicians, Pharmacy, and the business office. Additional duties as assigned. (Pay scale NG58) Telework available after 1 year of successful performance, if desired. Preferred Experience: Highly skilled in interpersonal effectiveness. (Customer service skills) Able to solve customer problems, treat customers with dignity and respect, acknowledges customers and responds quickly to customer's needs. Have one to two years of successful clinical nursing practice in an acute care setting preferred. Case Management experience is highly desirable.
