POSITION OVERVIEW : Wound Ostomy - Advance Practice Provider (Nurse Practitioner) provides medical and clinical leadership in collaboration with the medical director to the Wound Ostomy services department. This position will primarily cover the outpatient clinic and deliver inpatient consults as needed at other St. Charles Health System campuses. Responsible for wound evaluation to create comprehensive care plans, assigning treatment options and education to best fit patients' needs. Will work collaboratively with referring physicians and the care team to provide quality patient and family centered care.
DEPARTMENT SUMMARY: Our hospital-based, outpatient clinic provides the highest level of wound care and ostomy management. We serve patients with problematic and hard-to-heal wounds. Our clinic also provides ostomy care for patients with colostomy, ileostomy, urostomy, fistulas and leaking tube or drain sites.
ESSENTIAL FUNCTIONS AND DUTIES:
Responsible for wound and ostomy care and oversight for patients referred to the Wound and Ostomy department; including wound care assessment, implementation of wound care interventions, consultation with physicians, communication, and collaboration with members of the care team, and support organizational education activities promoting wound healing and preventing skin break down.
May perform additional duties of similar complexity within the organization, as required or assigned.

Complies with all applicable laws, regulations, policies, and procedures, supporting the health system's corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violations of applicable rules, and cooperating fully with all health system's investigations and proceedings
Keep all licenses and appropriate certifications current and unrestricted to maintain privileges at a St. Charles Health System hospital sufficient to permit performance of job duties
Increase understanding and collaboration throughout the medical community
Maintain compliance of hospital's Medical Staff Bylaws, Rules and Regulations, as required by SCHS Board of Directors
Document all patient care within the Electronic Medical Record (EMR) and complete all necessary tasks in compliance with health system policies and regulatory agencies to ensure timely and accurate billing.
Be familiar with the SCHS organization structure.
Keep medical knowledge and skills current through regular CME activity. Must attest to having adequate (CME) to meet maintenance of board certification and compliance with MSO rules and continuing privileges without restriction.
Keep all licenses and appropriate certifications current and unrestricted.
Maintain active privileges at a St. Charles Health System hospital sufficient to permit performance of job duties.

This includes introducing oneself and stating your purpose, asking the patient their preferred name, spending time with the patient to determine their care goals, active listening, and using skillful communications (appropriate touch, eye contact, etc.).

Successful completion of an approved Nurse Practitioner course of study and certification as appropriate to the incumbent's license
Attends Mandatory educational offerings as described in SCHS policies/procedures and/or as required by other regulatory bodies
Complete annual CME as required to maintain license and appropriate Board certification
Complete assigned Computer Based Learning modules in a timely manner
Complete annual fire/safety, etc. education in-services as required
Previous CME's focused on wound care and/or ostomy care
Unrestricted Nurse Practitioner licensure in the state of Oregon.
Nurse Practitioner requires AANC, NCC or AANP, AMCB certification.
Unrestricted ability to participate in Medicare / Medicaid, or other federal or state governmental health care programs
Valid driver's license and the ability to meet SCHS driving requirements
Current BLS certification required
If provider will practice sedation, moderate or deep, ACLS certification is also required
Additional certifications may be required based on privileges granted to provider by Medical Staff Services at SCHS Inpatient locations
Unrestricted Federal DEA number registered in Oregon
Wound Care Certification
Minimum of 2 years relevant experience as Nurse Practitioner
Wound care experience
Experience using EMR (Electronic Medical Records) software
Advanced written and verbal communication skills to effectively interact with a diverse population and professionally represent St. Charles Health System
Experience working in a team environment and proven ability to collaborate, manage facilitation and consensus building among health care professionals and agencies to achieve expected results
1 year clinical experience in a wound care clinic or acute care setting
Experience in evaluation and management of patients requiring inpatient and outpatient services
Epic EMR software experience
PERSONAL PROTECTIVE EQUIPMENT
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely
Must have excellent communication skills and ability to interact with a diverse population and professionally represent SCHS.
