Fairview Geriatric Services seeks a board-prepared/board-certified Gerontological, Adult or Family Advanced Practice ProviderSupervisor to lead a team of gerontological and adult nurse practitioners and physician assistants in the care of our patients in nursing homes, assisted livings and transitional care facilities throughout the metro area. The Advanced Practice Provider Supervisor will also work clinically, in collaboration with physicians, as part of an interdisciplinary team, engaging in independent decision making about the health care needs of their own patient panel.

Practice Details:

• Work in collaborative teams withMD and team coordinator to provide on-site care where people live .
• Develop standards, role optimization and development; monitor performance and evidence-based practice; and recruit, hire, orient and develop staff.
• Meet the unique and growing needs of complex, frail, and special needs patients in care communities.
• Engage in independent decision making about health care needs and provide health care services to individuals.
• Openings:1.0 FTE .
• Our providers use a multidisciplinary team approach in managing clinical care.
• Fully integrated Electronic Medical Record (EPIC).

• A qualified candidate will be a graduate of an accredited school of nursing and formal Adult/Geriatric or Family Nurse Practitioner or Physician Assistant training program.
• Have a current Minnesota license and be approved or eligible by the Minnesota Board of Nursing or Minnesota Board of Medical Practice for prescription writing privileges.
• Be eligible or certified through ANCC / AANP / NCCPA / MAPA / AAPA and meet threshold criteria/qualifications for Credentialing and Privileges.
• Ability to obtain and/or maintain DEA certification for the State of Minnesota.
• Experience working as an Advanced Practice Provider in similar setting coupled with leadership experience strongly preferred.
