As a Case Manager RN, you will have the best of both worlds. You have flexibility to work from home as well as provide member care in a direct care setting. This role will require in person visits (on occasion) to members in the Lubbock, Abilene, San Angelo, Midland/Odessa, and Amarillo areas.
If you are located in the West Texas area and able to commute within a designated area, you will have the flexibility to telecommute* as you take on some tough challenges.
Primary Responsibilities:
- Making outbound calls to assess members' current health status
- Visiting members in the hospital to prepare for transition home (varies by market)
- Identifying gaps or barriers in treatment plans
- Providing patient education to assist with self-management
- Providing clinic and provider education
- Interacting with Medical Directors on challenging cases
- Coordinating care for members
- Making referrals to outside sources
- Coordinating services as needed (home health, DME, etc.)
- Educating members on disease processes
- Encouraging members to make healthy lifestyle changes
- Documenting and tracking findings
- Making "welcome home" calls to ensure that discharged member receive the necessary services and resources

Required Qualifications:
- Current, unrestricted RN license in Texas
- 3+ years clinical experience in a hospital, acute care, home health/hospice, direct care or case management
- Experience working with clients in the community setting
- Must have a designated work space and access to install secure high speed Internet via cable / DSL in home
- Demonstrate a proficiency in computer skills including Microsoft Word, with the ability to navigate a Windows environment
- Access to reliable transportation
- Ability to travel up to 25% within a designated area

Preferred Qualifications:
- Bachelor's Degree
- Certified Case Manager (CCM)
- 5+ years of experience as an RN
- Case Management experience
- Experience / exposure with discharge planning
- A background in managed care
- Solid organizational skills and multitasking abilities will be keys to success
