The LPN Case Manager acts as a liaison for members&providers through the continuum of care,contact member/family to educate&coordinate with the interdisciplinary team. Establish a rapport utilizing a preventive approach, & assist in scheduling PCP transitional visit and participate in IDT patient care conferences.

Identify, coordinate and/or provide appropriate levels of care under the direct supervision of an RN or MD
Perform clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating). This includes case management, coordination of care and medical management consulting.
Provide health education, coaching and treatment decision support for members
Expect to travel about 20% of the time to meet with members in their homes, the hospital or in a long-term care setting.

High School diploma or GED or equivalent work experience
Unrestricted LPN or LVN license in the State where the position will be performed
Basic computer proficiency (Microsoft Word, Outlook and internet)
This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease
Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements.
