Atlantic Health System is seeking a Nurse Practitioner, Palliative Care Clinician at Morristown Medical Center. Will perform comprehensive health histories and physical assessments of patients living with serious illness; incorporate holistic aspects of the patients' care and accurate documentation of findings in a timely manner; employ skilled and meticulous symptom management, and continuum of care planning.
These services will be provided primarily in an embedded ambulatory/outpatient setting where the nurse practitioner will coordinate with the patient's primary care and specialist clinicians. Comfort practicing in the inpatient setting and post-acute facilities is an asset. In-person hours mostly scheduled during the weekdays with participation in an on-call coverage rotation worked remotely.

Essential Job Functions:
Conducts comprehensive, focused assessments of patients and families with palliative care needs.
Work with patients and families to determine their understanding of the patient's condition, proposed treatment plan and advance care planning.
Formulates, implements, monitors, evaluates, and modifies palliative care plans with input from an interdisciplinary palliative care team and other collaborating care providers (primary care and specialists).
Assist patients and families in developing treatment plans, including the management of medications and pain control.
Assist in all facets of care coordination for palliative care referrals.
Completes advance care planning with patient and family and refers to hospice as is appropriate.
Engage in quality improvement in patient care services and department functions, including evaluating the effectiveness of the health care delivery system in meeting the health care needs of palliative care patients and families.
Plans clinical and educational interventions, which are consistent with individual/organizational needs and goals, and evaluate change/outcomes.
Participates in on-call coverage rotation.
Partners in the development and growth of the Palliative Care Program.
Participates in palliative care research with a focus on outcomes.
Candidate will have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served. Through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.

Masters degree in Nursing required
Valid NJ NP license required
Palliative Care certification a plus
1-2 years experience preferred
