Position

The Social Worker functions as the key member of the health care team responsible for ensuring appropriate discharge planning ensured through assessment, (social, financial, and health conditions of patient) from which recommendations for referrals, services, and post hospital care will be provided. The Social Worker functions will be in collaboration with physicians, nursing staff, physical therapy and/or other members of the multi- disciplinary team as appropriate.

Education

• Graduate of an accredited school of professional Social Work (MSW) required. Candidates with a Masterâ��s degree in Business (MBA) and/or Health Administration (MHA) will be considered.

Skills and Abilities

• Able to communicate and negotiate with all levels of society. Ability to function independently within a team framework, as well as good organizational skills. Knowledge of community resources, reimbursement methodology.

Experience

• Three (3) years acute care experience as a Social Worker preferred.
